where can I find the completed 2099r forms for 2008?

You can find the template form 1099R at the IRS web site:

http://www.irs.gov/pub/irs-pdf/f1099r.pdf

For each pension, annuity, profit-sharing plan, retirement plan, IRA, and/or insurance contract that you get a distribution from - you should get a 1099R from each plan custodian by January 31st of the year following the distribution. So for 2008 - you should get this by January 31, 2009.
